To apply, first register with our site and have a log in account. Without an account in place, we are unable to upgrade you to premium membership.

After submitting your application, you will be directed to a page with our address and toll free fax number. A copy of your license is required to complete the application process.
Incomplete applications will not be processed.

Approved accounts will be upgraded and notified via email. Discounts are immediate and appear when you are logged in to your account. A current license is required to be on file. Accounts with an expired license will not receive discounts until a valid copy is submitted.
